Cholecystostomy is a procedure to drain the gallbladder, often done as an alternative to surgery or as a bridge to surgery for those with gallbladder disease.

What is Cholecystectomy?

Cholecystostomy is a procedure to drain the gallbladder, often done as an alternative to surgery or as a bridge to surgery for those with gallbladder disease. It involves creating an opening in the gallbladder and placing a catheter (tube) to drain bile and fluids. This procedure is typically used when a person is too sick or not a good candidate for surgery.

Types of Cholecystectomy:

Laparoscopic Cholecystectomy:

  • Minimally Invasive: Involves small incisions in the abdomen.
  • Laparoscope: A thin, lighted tube with a camera is inserted to provide a view of the gallbladder and surrounding structures.
  • Instruments: Special surgical tools are inserted through the incisions to remove the gallbladder.
  • Advantages: Shorter hospital stays (often outpatient), less pain, quicker recovery, and smaller scars.
  • Commonly Used: The preferred method for most cholecystectomies.
  • Open Cholecystectomy:

  • Traditional Approach: Involves a larger incision in the abdomen.
  • Access: The gallbladder is directly accessed and removed through the incision.
  • Used When: May be necessary in complex cases, when laparoscopic surgery is not feasible, or in emergencies.
  • Recovery: Longer hospital stay (2-3 days) and recovery time (4-6 weeks) compared to laparoscopic surgery.
  • Less Common: Less frequently used than laparoscopic surgery.
  • Symptoms:
  • Fever or chills.
  • Redness, swelling, bleeding, or other drainage from the incision site.
  • More pain around the incision site.
  • Yellowing of your skin or the whites of your eyes (jaundice)
  • Belly or abdominal pain, cramping, or swelling.
  • No bowel movement or gas for 3 days.
  • Pain behind your breastbone.

  • Procedure:

    Open Cholecystectomy:

  • Incision: A 6-inch incision is made in the abdomen, below the ribs on the right side.
  • Exposure: The muscle and tissue are pulled back to reveal the gallbladder and surrounding structures.
  • Dissection: The gallbladder is carefully separated from the liver and surrounding tissues.
  • Removal: The cystic duct and artery leading to the gallbladder are cut, and the gallbladder is removed.
  • Closure: The incision is closed with sutures or staples.

  • Laparoscopic Cholecystectomy:

  • Incision: Several small (1 cm) incisions are made in the abdomen, usually near the belly button and on the right side.
  • Inflation: Carbon dioxide gas is pumped into the abdomen to create space for the procedure.
  • Laparoscope: A thin tube with a camera (laparoscope) is inserted through one of the incisions to visualize the gallbladder.
  • Instruments: Surgical tools are inserted through the other incisions to manipulate and remove the gallbladder.
  • Removal: The gallbladder is removed through one of the incisions, often the umbilical incision.
  • Closure: The incisions are closed with sutures, staples, or adhesive.
  • Recovery:

    Recovery from cholecystectomy (gallbladder removal surgery) typically takes around 2 weeks for laparoscopic (keyhole) surgery and 6 to 8 weeks for open surgery. Return to normal activities may be possible within a few weeks, but it can take up to 6 weeks to fully recover your energy levels.
