Types of Ligament reconstruction:

Autograft: Uses a tendon from another part of the patient's body to replace the damaged ligament. Common autografts include patellar tendon, hamstring tendon, and quadriceps tendon.
Advantages: Lower risk of rejection, no donor site complications, and potential for faster healing.
Disadvantages: Possible donor site pain and delayed healing.

Allograft: Uses tissue from a donor (cadaver or live donor) to replace the damaged ligament.
Advantages: Avoidance of donor site pain and potential for faster healing.
Disadvantages: Higher risk of rejection, possible delayed healing, and potential for disease transmission.

Synthetic Graft: Uses artificial materials to replace the damaged ligament. Examples include LARS (Ligament Advanced Reinforcement System) and Gore-Tex.
Advantages: No donor site complications, potential for ease of surgery, and no risk of rejection.
Disadvantages: Potential for longer healing times, mechanical failure, and less natural integration with the bone.

  • Procedure:

    1. Anesthesia and Incision:
    The patient is given anesthesia, and the surgeon makes an incision to access the damaged ligament.

    2. Graft Preparation:
    A graft is harvested, either from the patient's own body (e.g., hamstring tendon or patellar tendon) or from a donor.

    3. Bone Tunneling:
    The surgeon creates tunnels in the bones surrounding the joint where the ligament was torn.

    4. Graft Placement and Fixation:
    The graft is threaded through the tunnels and secured in place with screws or other fixation devices.

    5. Joint Stabilization and Testing:
    The surgeon tests the joint's stability and range of motion to ensure the graft is properly fixed and the knee is functioning correctly.

    6. Closure:
    The incisions are closed, and a dressing is applied.

    Recovery:

    Recovery from ligament reconstruction surgery, like ACL reconstruction, typically takes 6 to 12 months, with athletes and those returning to sport potentially needing longer. The initial phase focuses on pain management, limited weight-bearing, and regaining range of motion. Subsequent phases involve strengthening exercises, progressing to running and pivoting, and eventually returning to sports.
